Appropriate use criteria will help physicians select treatments for patients

WASHINGTON, DC (January 30, 2012) Updated appropriate use criteria released today offer detailed guidance on when to use an invasive procedure to improve blood flow to the heart and how to choose the best procedure for each patient. The clinical scenarios, written by a group of cardiologists and cardiac surgeons, affirm the role of revascularization for patients with acute coronary syndromes and significant symptoms.

"This document helps patients, physicians, and payers determine when it's reasonable to do a procedure that is intended to improve the patient's quality of life, health status, and long-term survival," said Manesh Patel, M.D., the John Bush Simpson Assistant Professor of Cardiology at Duke University in Durham, NC, and lead author of the updated appropriate use criteria. "It can also assist patients and physicians with health-related discussions and shared decision-making, so that patients are confident they are getting the right procedures for them."

The updated appropriate use criteria replace a previous set published in 2009. Both documents were jointly developed by the American College of Cardiology Foundation, Society for Cardiovascular Angiography and Interventions, Society of Thoracic Surgeons, American Association for Thoracic Surgery, American Heart Association, American Society of Nuclear Cardiology, Heart Failure Society of America, and Society of Cardiovascular Computed Tomography. They have been endorsed by the American Society of Echocardiography and Heart Rhythm Society.

When developing the 2009 appropriate use criteria, the writing group identified nearly 200 clinical scenarios that reflect common heart problems seen in everyday cardiology practice. These scenarios were rated by an expert panel composed of interventional cardiologists, heart surgeons, non-interventional cardiologists, other physicians who treat patients with cardiovascular disease, health outcomes researchers, and a medical officer from a health plan. Panelists assigned a score using a modified Delphi exercise to each scenario indicating whether an invasive procedure to restore blood flow to the heart would be considered appropriate, inappropriate, or uncertain. An essential component in evaluating each clinical scenario is whether clinical studies have shown the procedure would be likely to improve patients' survival or quality of life. The rating takes into account such factors as symptoms, medication, results of stress testing, severity of disease burden, and number of coronary blockages.

New clinical data and gaps identified in data collection spurred the update of the appropriate use criteria, Dr. Patel said. "This update provides a reassessment of clinical scenarios the writing group believed to be affected by significant changes in the medical literature or gaps from prior criteria," he said. "For example, publication of the SYNTAX trial called for the reexamination of clinical scenarios for multi-vessel coronary artery disease, and implementation efforts revealed a few scenarios not captured in the 2009 publication."

When a patient's coronary arteries become clogged with deposits of cholesterol and fat, the heart muscle may not get enough blood flow to function properly. The result can be chest pain or even a heart attack. If medication and lifestyle changes alone are not enough, physicians and patients must decide whether to improve blood flow with bypass surgery or catheter-based angioplasty and stenting. That decision is based not only on a patient's overall health and personal preferences, but also on which arteries are clogged and how severely.

Symptom status, extent of medical therapy, risk level as assessed by noninvasive testing and coronary anatomy all play a critical role in decision making. The appropriate use criteria scenarios were developed to mimic patient presentations encountered in everyday practice and to address the rational use of coronary revascularization in the delivery of high-quality care. Appropriate revascularization procedures can improve patients' clinical outcomes, whereas inappropriate revascularization exposes patients to unwarranted risk and increases costs to the health care system.

Peter K. Smith, M.D., Professor and Chief of Cardiothoracic Surgery at Duke University and co-author of the new criteria, said, "The new ratings that have changed PCI from inappropriate to uncertain for low burden left main disease, and from uncertain to appropriate for low burden three-vessel disease should result in careful selection of high-risk surgery patients for PCI. Surgeons and cardiologists will now work together to maximize the benefit and minimize the risk for these patients who are at high risk for premature mortality."

The updated criteria reaffirm that coronary artery bypass is appropriate for patient scenarios with coronary artery disease involving two vessels to include the proximal LAD and all variations of three-vessel and left main coronary artery disease. PCI is appropriate in patients with coronary artery disease in all three heart arteries only if the severity of coronary artery disease burden is low. It is uncertain whether PCI is appropriate in patients with three- vessel coronary artery disease and an intermediate to high disease burden. The appropriateness of PCI is also deemed uncertain in patients with blockages in the left main coronary artery, alone or with blockages in other arteries and low coronary artery disease burden. However, PCI is considered inappropriate in patients with blockages in the left main coronary artery with intermediate to high disease burden, according to the new criteria.

"A rating of uncertain does not mean that it's not reasonable or should not be done," Dr. Patel said. "These are areas where we don't have definitive evidence and think more research is needed. Remember, the majority of medicine may be considered uncertain by this evidence standard for improvement in health status or longevity. That's where physicians apply their clinical experience and knowledge to patient care and patients express their wishes."

Dr. Patel said the new appropriate use criteria will stimulate discussion regarding appropriate use of revascularization: "We hope physicians, surgeons, caregivers, and patients have a conversation in each individual case about the best way to improve patient outcomes."

The appropriate use criteria will be published in the February 28, 2012, issue the Journal of the American College of Cardiology and will be available before print on Monday, January 30, at 2 pm ET at www.cardiosource.org. Significance He is a prosecutor, one who is actually responsible for the formation of the special investigation team. His identity as a vampire is secret to those around him and he survives because of a friend in the medical field who gives him blood-bags from people who donated blood. He formed the special team in order to catch Xia, a young vampire moved to New York City five years ago in hopes of finding a life-long companion.
History Much of his past before he was turned is a blur and as time has continued on, he has forgotten exactly how old he really is. He is constantly on the move, changing jobs and identities in order to locate those of his kind and keep them from exposing the truth about vampires. He truthfully believes that Xia is the woman who turned him into a vampire, but he cannot be sure.
Personality Abel is a guy who few try to grow close with. He is very mysterious, showing only a blank face and a smile that says he knows more than he tells. Rarely wasting a moment to joke, his seriousness is a plague that people try to avoid. In truth, he is very lonely, though his only true friend is his doctor friend who provides him with blood, and even he doesn?t understand him fully.
Other Like the vampire prosecutor, he is also strongly drawn to the female prosecutor?s blood.

Significance She is the one responsible for the ?vampire attacks? five years ago. Lately she has been laying low, however, only making kills every now and then to gain the attention of the vampire prosecutor, who she has fallen in love with. Every now and then she will visit him out of the blue or will call his phone and present him with information that helps him with his cases. Her attacks have been in precise locations. If you were to dot them out on a map and then connect the dots, they would be forming an arrow. This arrow is pointing to the part of the city where she is residing and where she awaits the arrival of the vampire prosecutor.
History For many years, Xia has been moving from place to place. Seven years ago while in Los Vegas, she ran into Abel who was visiting relatives. Impressed with his abilities, she turned him into a vampire. However, his response to the change angered her for he wasn?t overjoyed, but horrified. In her anger, she lost track of him. It was two years later when she found him in New York City. In order to gain his attention, she went on a mass killing spree. During the five years up until today, she has been diligently searching for a life-long companion; a mate. In the process she discovered the prosecutor. She turned him into a vampire, hiding her identity in order to force him to find her.
Personality You can sum up Xia in one word: self-centered. She is only out for herself, not thinking of how her actions can affect the lives around her. She lives for the moment and thrives for pleasure ? hence why she is searching for a mate. Because she is on her own, her frustration and anger often takes form of a vampire attack. She loves to play with people?s emotions and will do anything to get what she wants.

Significance Neo is the guy that provides the vampire prosecutor the blood he needs. He is the owner of a club named Club Blood ? this is where he stores the blood. Generally he also provides insight and advice while the two sit to drink. Though he isn?t the only one who knows of the prosecutor?s vampire situation, he is constantly looking out for the guy and will try in every which way to help if needed.
History After working for several years as a doctor, Neo retired from his job. He and a friend had a serious argument over disappearing blood bags from the donation branch and in the end, he decided he was done. Wealthy from his years of working, he opened the club. (will add part of meeting the prosecutor later)
Personality Straight forward and honest, Neo is a friendly character. There aren?t many people who dislike him and vice versa. He is generally viewed as a fatherly figure and does his best to help those in need. However, this fatherly figure disappears when someone he treasures is threatened. He suppresses a dark side, one he developed after working as a depressed doctor for many years.
Other His wife died in a massive car accident two years ago.

TOKYO (Reuters) ? Japan's population is expected to fall by 30 percent to below 90 million by 2060, with two out of every five people 65 or older, a government agency said on Monday, underlining the financial burden looming over the fast-aging society.

The grim forecast underscores the failure of efforts to encourage people to have more babies and will add urgency to efforts on tax and social security reform, and could also stir debate on immigration.

By 2060, the number of people aged 14 or younger is forecast to be less than 8 million while there will be nearly 35 million people aged 65 or older.

The population is aging at the fastest pace among developed countries because of a low birthrate and long life expectancy.

"The trend of the aging society will continue and it is hard to expect the birth rate to rise significantly," Chief Cabinet Secretary Osamu Fujimura told a news conference.

"Thus, comprehensive tax and social security reform is needed."

Prime Minister Yoshihiko Noda has vowed to double a 5 percent sales tax in two stages by October 2015 to help fund bulging social security costs, which are rising by 1 trillion yen ($13 billion) a year and aggravating a public debt already twice the size of Japan's $5 trillion economy.

But the biggest opposition party, although agreeing on the need for a tax increase, is threatening to block legislation in parliament's upper house. The opposition argues that the ruling Democrats' plan to revamp public pensions would require a higher levy than planned.

The population is expected to fall below 100 million in 2048 and dip further to 86.74 million by 2060, from 128.06 million in 2010, according to a projection by a research arm of the Health Ministry.

By 2060, the number of people aged 14 or younger is forecast to fall by more than half, to 7.91 million. By contrast, the number aged 65 or older is seen rising 18 percent to 34.64 million, accounting for 39.9 percent of the population, compared with 23.0 percent in 2010.

The pace of aging has slowed somewhat from the previous estimate made in 2006 but the overall trend has not changed.

"URGENT AND VITAL ISSUE"

The fertility rate, the expected number of children born per couple, is expected to reach 1.35 in 2060 from 1.39 in 2010, below the 2.08 needed to keep the population from shrinking and compared with a global rate of about 2.5.

Some experts have called for the liberalization of immigration rules but many Japanese are cautious about opening the door to foreigners who, many believe, have trouble assimilating in a society where homogeneity has been seen as a source of stability.

Hidenori Suezawa, chief strategist at SMBC Nikko Securities, said the government needed to tackle the sluggish birth rate to help fix the nation's tattered finances and social security system.

"It is necessary to reaffirm that policies to address the low birth rate are an urgent and vital issue," he said in a note.

Japan has been trying for at least two decades to raise the birth rate without success and critics say making it easier for women to work and raise children at the same time is key.

The ruling Democrats promised to beef up childcare allowances when it took power for the first time in 2009 but it has had to revise that pledge because of a worsening fiscal problem.

The population estimate also showed that average life expectancy will rise by more than four years in 2060, to 84.19 for men and 90.93 for women.

The population projection is compiled roughly every five years based on data including a census and demographic statistics and serves as a reference for government social security policy.

Police focus on SUV in fatal N. Calif. train crash (AP)

SACRAMENTO, Calif. ? Investigators on Sunday were trying to determine what motivated the driver of a sport utility vehicle to ignore a downed crossing arm and flashing lights and pull the vehicle into the path of an oncoming commuter train in Sacramento.

That video and other pictures captured by a camera mounted on the train are part of the investigation and were not being released publicly, Peck said.

Witness accounts appear to support the video evidence that the crossing arms were down and warning lights were flashing when the SUV tried to get across the tracks.

Davis resident Ravin Pratab, 42, was in a car that was waiting to cross the tracks when he said he heard a loud bang and then "saw a light-rail train heading south with a big truck smashed on it."

Authorities said six of the roughly 50 passengers on the light rail train were taken to local hospitals but had only minor injuries.

On Sunday, the tracks were cleared and the intersection was open, with no sign of the previous day's collision. A white teddy bear was placed at the base of the pole holding the crossing arm, on the same side of the tracks where the SUV had been before it attempted to cross.

Regional transit officials said trains were operating on their regular schedule after a section of track was repaired Saturday night.

One question investigators are trying to answer is the length of time the crossing arms were down. The light rail train passed through the intersection after two Union Pacific freight trains, going in opposite directions and using different tracks, had passed by.

Neither Peck nor a spokeswoman for the transit district said they knew the length of the interval between the time the freight trains cleared the intersection and the commuter line came through. The light rail system has its own dedicated tracks.

Drivers in Sacramento often can wait up to 10 minutes for a freight train to pass, then might have to wait several minutes more because of an approaching light rail train. The extended wait times can be a source of irritation ? and missed appointments ? in California's capital.

Joe Paterno died Sunday of lung cancer at age 85.

At a memorial service Thursday that drew some 12,000 people to the Penn State basketball arena, Jay Paterno reflected on what he called the "magnificent daylight" of his legendary father's life. It was primarily a glowing tribute to Paterno and his accomplishments during 46 years as Penn State's football coach ? but also an opportunity to defend his legacy against criticism that he failed to do more when told about an alleged child sexual assault involving one of his former assistants.

Nike founder and CEO Phil Knight won a thunderous standing ovation when he defended Paterno's handling of the 2002 allegations against former defensive coordinator Jerry Sandusky. Paterno, he hinted, had been made a scapegoat.

"If there is a villain in this tragedy, it lies in that investigation and not in Joe Paterno's response," Knight said. Paterno's widow, Sue, was among those rising to their feet.

Capping three days of mourning on campus, the 2?-hour ceremony was filled with lavish praise for the man called "JoePa." Paterno racked up more wins ? 409 ? than any other major-college football coach, led his team to two national championships, and preached "success with honor" while insisting his athletes focus on academics. The Paternos donated millions to Penn State.

Though the campus and surrounding community have been torn with anger over the Sandusky scandal and Paterno's summary dismissal by the board of trustees two months before his death, Jay Paterno said his father didn't hold a grudge.

"Despite all that had happened to him, he never wavered in his belief, in his dream, of Penn State. He told me he wanted to use his remaining time on earth to see Penn State continue to thrive. He never spoke ill and never wanted anyone to feel badly for him," Paterno said.

Players from each decade of Paterno's career as the Nittany Lions' coach spoke in loving terms about their mentor, saying he rode them hard, but always had their best interests at heart and encouraged them to complete their educations and become productive members of their communities.

Among the speakers were Michael Robinson, who played for Paterno from 2002 to 2005 and flew in from Hawaii, where he was practicing for his first Pro Bowl; star quarterback Todd Blackledge from the 1980s; and Jimmy Cefalo, a star in the 1970s. Like Robinson, Blackledge and Cefalo went on to play in the NFL.

Former NFL player Charles V. Pittman, speaking for players from the 1960s, called Paterno a lifelong influence and inspiration.

Pittman said Paterno challenged his young players, once bringing Pittman to tears in his sophomore year. He said he realized later that the coach was molding him into the man he would become.

"What I now know is that Joe wasn't trying to build perfection. That doesn't exist and he knew it. He was, bit by bit, building a habit of excellence," said Pittman, now a media executive on the board of The Associated Press.

Paterno was fired Nov. 9 after he was criticized for not going to police in 2002 when he was told that Sandusky had been seen sexually assaulting a boy in the showers. Sandusky was arrested in November and is awaiting trial on charges that he molested 10 boys over a 15-year span.

As the scandal erupted, Pennsylvania's state police commissioner said Paterno may have met his legal duty but not his moral one. Penn State president Graham Spanier was also fired in the fallout.

Knight, appearing about midway through the memorial, became the first speaker to explicitly address the scandal. He said the coach "gave full disclosure to his superiors, information that went up the chains to the head of the campus police and the president of the school. The matter was in the hands of a world-class university, and by a president with an outstanding national reputation."

Lanny J. Davis, an attorney for the board, responded after the service by saying: "All the reasons for the board's difficult and anguished decision ? made unanimously, including former football players and everyone who still loves Coach Paterno and his memory ? reached a decision which was heartfelt. All 32."

"The facts speak for themselves" and include the grand jury testimony, he said.

Chris Marrone, another former player who eulogized Paterno, said Knight was his "new hero" for expressing the "pent-up frustration" of Paterno's supporters.

"I think the response that he got is indicative of how folks feel," Marrone said.

Only one member of the university administration ? the dean of the college of liberal arts ? and no one from the board of trustees spoke at the memorial, which was arranged primarily by the Paterno family.

People said it felt good to remember and celebrate the good times.

Tennessee Titans coach Mike Munchak, who played for Paterno, said he attended the service "because I'm a part of his legacy."

"It was not only about football," Munchak said. "It was about life and how he affected all of us as men."

Department of Education names finance leader

The Arkansas Department of Education has hired the superintendent of the Lavaca district in Sebastian County as its new assistant commissioner for the Division of Fiscal and Administrative Services.

Jared Cleveland will begin the new role on July 1.

"Jared has a complete understanding of how schools operate," Arkansas Education Commissioner Tom Kimbrell said in a news release. "He knows school district issues well and has been directly involved in many of the complex policy and financial problems that affect them. We are fortunate to have someone of his caliber who is experienced with the intricacies of Arkansas's school finance system."

Cleveland previously was a superintendent, high school principal, coach and teacher in the Magazine district in Logan County.

Kathleen Crain has been filling the department of education's finance position in an interim capacity since Bill Goff left for a job in the Pulaski County Special School District.

Britain ranks top risks posed by climate change

(AP) ? Coastlines, working patterns, and even the country's most famous meal are under threat from climate change, Britain said Thursday in its first-ever national assessment of the likely risks.

The 2.8 million pound ($4.4 million) study sets out the most pressing problems expected to affect the United Kingdom as a result of climate change, from rising sea levels to more frequent summer droughts.

In a gloomy forecast for Britain's environment department, a panel of independent analysts predicted as many as 5,900 more people could die as a result of hotter summers ? but also claimed there will be a sharp reduction in deaths currently due to cold weather by the 2050s.

Infrastructure and businesses will be badly affected by more frequent floods, with the cost of damage likely to rise from 1.3 billion pounds ($2 billion) to as much as 12 billion pounds ($18.8 billion) by the 2080s, if adequate preparations aren't taken.

By the 2050s, between 27 million and 59 million people in Britain are likely to be living in areas suffering problems with water supplies, the report claims. Britain is predicted to have a population of about 77 million by 2050.

Beaches and historic coastlines are likely to be reshaped by coastal erosion, with the rate expected to increase fourfold, the report said. "This might have significant implications for communities and habitats," it said.

Analysts predict an increase in the overheating of workplaces which would harm businesses by reducing employee productivity and increasing energy bills, because of a greater reliance on air conditioning.

Without alteration work, sewers will overflow more frequently and spill pollution into seas and rivers, while heavier rainfall is likely to cause frequent damage to roads, railway tracks and bridges.

The report also warned that Britain's stocks of cod ? a key component of the nation's beloved fish and chips ? will dwindle, but should be replaced by more plentiful numbers of fish such as plaice and sole.

However, the study also points out possible benefits to Britain. It notes that there will likely be better yields for crops of wheat, sugar beet and potatoes, that the melting of Arctic sea ice will open quicker shipping routes and that warmer temperatures will make the U.K. a more attractive tourist destination.

"Without an effective plan to prepare for the risks from climate change the country may sleepwalk into disaster," said John Krebs, chairman of a group that advises Britain's government on adapting to climate change.

Britain has pledged to cut its carbon emissions in half by 2025, though the target could be loosened if other European countries fail to cut their own emissions accordingly.

The U.K. has a legally mandated commitment to reduce greenhouse gas emissions by 60 percent by 2030, and 80 percent by 2050.

Analysts who drafted the report said other nations likely face more significant challenges than Britain in coping with the impact of a changing climate.

"Potential climate risks in other parts of the world are thought to be much greater than those directly affecting the U.K., but could have a significant indirect impact here ... on global health, political stability and international supply chains," the report said.
